Within 45 days after the event (if application is accepted), a follow up report of this event is required providing the following:

  1. please use the follow up report form that will be enclosed with the letter stating that your grant application is approved.
  2. a COPY of all receipts (signed) covering expenses (original receipts will not be returned)
  3. a master sheet (included with the acceptance letter) with names, addresses, gender & age group including the caller(s) / cuer(s) / instructors and their partner(s).   It can be used as sign in sheets or just filled in by typing or writing the names, etc.    Keep the signed lists for your records.   Whatever works for you is acceptable.

All promotions of the event must have the Saskatchewan Lotteries Logo displayed.  Contact Bernice for copies of logo if you do not have any.
